New Patients

Welcome! Thank you for choosing Integrative Behavioral Health and Healing Practice as your partner in your mental health and wellness journey. Here’s what you need to know to establish care with us.  

Most providers offer telehealth New Patient appointments. There are in person follow-up visit options with Mary Hartsell and Karen Lavieri once you are an established patient. 

Provider Karen Lavieri has both in person New Patient appointment availability as well as telehealth. 

Follow these steps to schedule your first appointment with your new Integrative Behavioral Health and Healing Practice provider.

Step 1: Complete the New Patient Paperwork

Before you can schedule your first appointment, the following packet must be completed, submitted and accepted by our office. Please complete ALL pages of the packet by clicking the link provided below. You will be able to submit all the New Patient forms listed below as one PDF document. In order to process your paperwork in a timely manner, please be sure to complete each section in its entirety.  

We strongly encourage these forms to be filled out using a desktop computer, laptop computer, or a tablet for ease of use, as the forms may not be mobile friendly. 


The New Patient Packet includes: 

– Patient Information 

– Medical History 

– Medication List 

– Practice Policies 

– Telemedicine Patient Agreement 

- Controlled Substance Prescription Policy

– Notice of Privacy Policies Acknowledgement of Receipt 

 – Credit Card Information 


Note: Completing the forms online will route you to a secure fillable form. Once you have reviewed and completed all pages, you will click the Continue button and confirm that you want to submit. Once the document has been processed and submitted to us, you will see a Thank You page that will indicate you have submitted the “New Patient Packet.” Your forms packet will be delivered to us electronically through a secure document portal. You will also receive a copy of the completed forms.  

 *Failure to complete the paperwork in full will delay scheduling an appointment. 

Step 2: Submit your Insurance Card and Photo ID

You are given the option to attach your insurance card and ID in the new patient paperwork, however if you are unable to do so, please email a clear legible copy of the following to us via email at info@integrativebehavioralhp.com  

– The front AND back of your insurance card 

– Your driver’s license (or other current photo ID) 

All patients must submit a copy of their ID and a current valid insurance card to provide proof of insurance. We cannot schedule an appointment until we have these. 

If you are insured by an insurance plan we contract with but do not have an up-to-date insurance card, payment in full for your visit will be required until we can verify your coverage. Please contact your insurance company with any questions you may have regarding your coverage. 

Step 3: Scheduling an Appointment

Once all required New Patient Forms and documents listed in Step 1 and 2 have been submitted and accepted by our office, we will contact you within 48 business hours to schedule your first appointment as a New Patient. 

Please note: New Patient Appointments are offered via telehealth Monday-Thursday 7:40am-4:00pm, Fridays between 8:00am - 12:00pm. Also, in person new patient appointments are offered on Mondays and every third Friday morning in our Raleigh office with Karen Lavieri.

Step 4: The day of your New Patient appointment

For telehealth:  

There are two ways to access your telehealth appointment. 

1 - Use the link included in your appointment reminder

2 - After accepting the patient portal invitation, you can navigate to your calendar and join the visit directly from the appointment listed.

Log in to your appointment waiting room for the provider you are scheduled with AT LEAST 10 minutes prior to your scheduled appointment. Please ensure you have a working electronic device (computer, tablet, or smartphone) and a working internet connection.   

For in-person: 

For in person new patient appointments with Karen Lavieri, arrive AT LEAST 10 minutes prior to your scheduled appointment and have a seat in the waiting room. She is in the Raleigh office at 920 Paverstone Dr, Ste D, Raleigh, NC 27615.
