Step 1: Complete the New Patient Paperwork

Before you can schedule your first appointment, the following forms packet must be completed, submitted and accepted by our office. Please complete ALL 13 pages of the forms by clicking the link provided below. You will be able to submit all the New Patient forms listed below as one PDF document. In order to process your paperwork in a timely manner, please be sure to complete each section in its entirety.  

We strongly encourage these forms to be filled out using a desktop computer, laptop computer, or a tablet for ease of use, as the forms may not be mobile friendly. 

The New Patient Packet includes: 

– Patient Information 

– Medical History 

– Medication List 

– Practice Policies 

– Telemedicine Patient Agreement 

– Credit Card Information 

– Notice of Privacy Policies Acknowledgement of Receipt 

 

Note: Completing the forms online will route you to a secure Adobe fillable form. Once you have reviewed and completed all 13 pages, you will click the blue Submit button, enter your email address and click Submit. Once the document has been processed and submitted to us, you will see a You’re all set page that will indicate you have submitted the “New Patient Packet.” Your forms packet will be delivered to us electronically though a secure document portal. You will also receive a copy of the completed forms.  

 *Failure to complete the paperwork in full, will delay scheduling an appointment. 

Step 2: Submit your Insurance Card and Photo ID

Please email a clear legible copy of the following to us via email at info@integrativebehavioralhp.com  

– The front AND back of your insurance card 

– Your driver’s license (or other current photo ID) 

All patients must submit a copy of their driver’s license (or other current photo ID) and a current valid insurance card to provide proof of insurance. If you are insured by an insurance plan we contract with but do not have an up-to-date insurance card, payment in full for your visit will be required until we can verify your coverage. Please contact your insurance company with any questions you may have regarding your coverage. 

Step 3: Scheduling an Appointment

Once all required New Patient Forms and documents listed in Step 1 and 2 have been submitted and accepted by our office, we will contact you within 48 business hours to schedule your first appointment as a New Patient. 

Please note: New Patient Appointments are offered via telehealth Monday-Thursday 7:40am-4:00pm; in person on Wednesdays mornings in our Raleigh office (Six Forks Rd) and on Thursdays in our Chapel Hill office (E Franklin St)

Step 4: The day of your New Patient appointment

For telehealth:  

Log onto the virtual Doxy waiting room for the provider you are scheduled with AT LEAST 15 minutes prior to your scheduled appointment. Please ensure you have a working electronic device (computer, tablet, or smartphone) and a working internet connection. Please note that if you are using a smartphone or a tablet for your appointment, after you sign into the waiting room using your name, if at any point your phone’s screen goes to sleep or you close the web browser window to access another app, you will be timed out on our end. To prevent this from occurring, every few seconds tap your screen until the visit begins.
